In the unfortunate event your child has an emergency room visit or hospitalization due to illness or injury, she/he must be cleared to return to school.

The physician who treats your child must provide you with a note upon discharge.

It is of the utmost importance for your child to submit this note to the nurse when they return to school. He/she will not be permitted to participate in gym or school activities without instructions from the doctor who treated your child. Any restrictions must be documented. If your child has no restrictions, it must be documented as well. A verbal order does not qualify. It would be helpful to remind the physician treating your child to provide the above at discharge.
