Religious/Medical exemptions to immunizations

Public Health Law requires all students have a complete set of immunizations for school attendance

 As of June 13th, 2019, NYS law states there is no longer a religious exemption to the requirement that children be vaccinated to attend school in a public, private or parochial school setting pre- kindergarten through 12th grade. 

If a student has a medical reason why they are unable to acquire vaccines, then a medical exemption form must be filled out by the student's physician and submitted to the school nurse. The medical exemption will be kept in the student's health record for the remainder of their SHS career.

It is to include:

  1. Complete information (name, DOB etc.).
  2. The vaccine or vaccines the medical exemption is referring to.
  3. Complete contraindication/precaution information.
  4. Complete date exemption ends, if applicable.
  5. Complete medical provider information.

In the unfortunate event there is a case of any disease at school, the medically exempt student will be excluded from school until the situation is resolved.
