Senior Autobiography: The senior autobiography is a required document completed by all seniors for their school counselor in the Fall of senior year.  The information shared in the document assists the counselor in writing the student's letter of recommendation for college, employment, military, etc.  You want to be thoughtful in how you answer the questions on the form and provide more than one sentence responses.

Parent/Guardian Brag Sheet: The parent/guardian questionnaire (brag sheet) is a required document to be completed by the parent/guardian of a senior in the Fall of senior year.  This form allows parents/guardians to provide insight about their child that will assist the counselor in writing a letter of recommendation.

Activity Resume Worksheet  The activity resume is a required document completed by all seniors for their school counselor in the Fall of senior year.  It is also a document seniors will need to share with their teachers when they request a letter of recommendation as well as information they will need to provide to colleges/universities and scholarship committees. 

Request for Teacher Letter of Recommendation: The teacher letter of recommendation request form must be completed one form per teacher. Traditionally, students will ask two teachers from different disciplines.  You want to be thoughtful in how you answer the questions on the form and provide more than one sentence responses.

Teacher Letter / College Application Timeline for Class of 2024

SHS College Application Processing Form: The SHS College Application Processing Form is a required form that must be filled out completely and given to your school counselor (either via email or in person) at least 10 school days prior to the college/university application deadline date.  Students should not submit their college/university application until they have meet with their school counselor and reviewed their application for accuracy prior to submission.

Official SHS Transcript Request.  This form gives the counseling department permission to send a student's transcript to a post secondary school or agency.
